Details of the 33-year-old actress’ fatal accident emerged Thursday, three weeks after her tragic death at Lake Piru, outside Los Angeles.
The certificate also revealed that the Glee star was buried last Friday at the famous Forest Lawn Memorial Park cemetery in the Hollywood Hills.

Naya went missing on Wednesday July 8 while visiting Piru Lake with her four-year-old son Josey.
That evening he was found asleep in his life jacket on their pontoon but there was no sign of the 33-year-old.

Police say it appears Rivera “gathered enough energy to get her son back to the boat, but not enough to save himself,” after confirming the star’s body had been found.

“We are very grateful for the outpouring of love and prayers for Naya, Josey and our family over the past week,” the family said. “As we mourn the loss of our beautiful legend, we are fortunate to honor his eternal legacy and his magnetic spirit. Naya was an incredible talent, but was an even bigger person, mother, daughter and sister.
